Case Management

Community Counseling Center provides Case Management to all clients at the onset of treatment, and the service may be ongoing as needed. Additionally, clients may access CCC’s Resource Room if they are in need of clothing, shoes, toiletries, or other basic necessities. Medical Case Management is available to any Ryan White Part A-funded clients who meet eligibility criteria. General Case Management services include:

• Applying for health insurance/Medicaid
• Finding a medical provider
• Applying for a Social Security Card
• Building a resume or searching for jobs
• Developing job interview skills
• Obtaining an outfit for a job interview
• Earning a GED or linkage to vocational training
• Locating daycare for children
• Paying for housing/utilities
• Securing food for yourself or your family
• Referrals to other community organizations
